When a claim arises, we want all of our clients to have immediate and responsive service. To help you in this process, please be reminded of the following few tips:
- Don’t admit fault
- Gather as much information from the other party as possible
- Write down names and phone numbers of any witnesses
- Contact our office as soon as possible
When you have a claim to report, select one of the following methods to report the claim to our office:
- You can reach us in the office to report a claim Monday through Friday, 8 am to 4:30 pm EST at (513) 271-5070 or toll free, (800) 382-2639 or fax (513) 271-4570
- If you can’t reach us in the office, click the appropriate form below. Complete the form online, hit the submit button and the information will be emailed to our office. We will promptly forward your claim to your insurance company.
